By submitting this Form, you hereby agree that Sunway Medical Centre Sdn Bhd may collect, obtain, store and process your Personal Data that you provide in this form for the purposes of event communications, administrative processing, and Continuing Professional Development (CPD) point verification, in addition to your receiving updates, news, promotional and marketing mails or materials from Sunway Medical Centre Sdn Bhd.

Personal Data relates directly or indirectly to you, who is identified or identifiable from that data, or
from that and other data in your possession, which includes your personal information to establish your identity and background, such as your full name, passport or identity card number, nationality, and religion.

You hereby give your consent to Sunway Medical Centre Sdn Bhd to:-

• Store and process your Personal Data;
• Disclose your Personal Data to the relevant governmental authorities or third parties where required by law or for legal purposes; and
• Use your Personal Data in such ways as to not constitute an invasion of your privacy.

For the purpose of updating, correcting, and/or removing such data, you may at any time apply or request to Sunway Medical Centre Sdn Bhd to access, modify, and/or retract your Personal Data stored by Sunway Medical Centre Sdn Bhd.

For the avoidance of doubt, Personal Data includes all data defined within the Personal Data Protection Act 2010, including all data you had disclosed to Sunway Medical Centre Sdn Bhd in this Form.
